Registering Your Team for the
2019 Oregon HS State League
**Registration due by March 1st**

In order to participate in the Oregon HS State League, your team needs to meet the following criteria:
- Have a coach with an official USAU Coaching Membership
- All players must have one of two types of USAU Memberships:
- Full Youth Membership ($38): For teams that will participate in the State Championship tourney
- Affiliate Youth Membership ($13): For teams that won’t participate in the State Championship tourney
3 Ways to Register
Depending on how prefer to run your team logistics, there are several different paths for meeting criteria above and signing your team up for the league. To determine which path is best for you, the key question revolves around whether you want to have your players/parents sign themselves up for USAU accounts or whether you, the coach or organizer, want to gather all the information and then sign everyone up in bulk. One extreme requires a lot of data collection on your part, while the other requires players/parents to navigate the complex maze that is the USAU website :).
Here’s an overview of the options:
A: Registering & Paying for Players in Bulk
We’ve worked with USAU to establish a very easy, streamlined way to register all your players (new and returning) for USAU memberships, roster them for the league, and pay in one simple step. You, the coach or organizer, will be responsible for gathering the necessary information needed to set up the account (DOB, email, name, etc), and compiling it on an official spreadsheet. Then USAU will do the rest (besides signing waivers and medical authorization forms). You will get a single invoice for all your player memberships (new and renewed) that can be paid all at once.
B: Players/Parents Create or Renew Memberships, but You Pay in Bulk
This option is for teams that want to direct their players/parents to sign up for their own USAU memberships, but then pay for all of them in bulk. In this scenario, we have simple instructions that you can circulate about how to sign up for a USAU account. Then once all the accounts have been created, you’ll get a single invoice for all your player memberships (new and renewed) that can be paid all at once. In this scenario, you’ll also be responsible for creating or rolling over your team in the USAU database and adding all the players (whereas in Option A, USAU does this for you).
C: Players/Parents Create/Renew and Pay for their own Memberships
In this scenario, you as the coach or organizer will direct all your players and parents to sign up and pay for their own USAU membership. Then you, as the coach/organizer, will need to set up your team and then add all players to the roster.
In all three scenarios, there is an option for players to apply for the Play It Forward Program, which allows qualifying individuals to have their membership fees fully covered by USAU.
Once you choose which option is best for you, you can follow the step-by-step instructions below to get your team registered:
Option A:
Registering and Paying for Players in Bulk
1: Set Up Your Coaching Membership(s)

All coaches must sign up for an official USAU coaching membership. This requires that they pass a background check in addition to undergoing several other online trainings/certifications.
Here’s the Process:
- If you have had a coaching membership in the past, are up to date on your certifications and background checks, and just need to renew your membership:
- There should be an alert above all the icons that shows your membership status
- Click the “renew membership” button and follow the prompts
- If you haven’t qualified for a coaching membership before:
- Login to your USAU account
- Go to your account homepage
- There should be an alert above all the icons that says something like “Your Coach Membership Status: Click here to view or update.” If you don’t see that then click “renew your membership” and select the “Coach” or “Coach and Player Options.” This will trigger an alert that says something like “You haven’t met the requirements” and send you through to the coaching status page
- Once you’re on the coaching status page, you’ll see a list of all the “things you need to do.” Just follow this list and the associated instructions and you’ll be good to go:
- Take the Safe Sport Training Video: Free and takes 90 minutes
- CDC’s HeadsUp Concussion Training Protocol: Free and takes 1 hour
- Background check: Costs $86.25
- USAU Ultimate Training: Free, and takes 45 minutes
3: Registering Qualifying Players for the Play It Forward Financial Aid Program
USAU has a program for low-income youth who qualify for their school lunch program. Essentially, players need to apply, proving that they are indeed receiving aid at school, and their USAU Youth Membership fee will be fully waived.
Have your players fill out this form and submit it to their coach/organizer who can then pass all applications on to USAU
5: Paying For Your Memberships in Bulk
Once you’ve completed your full roster, USAU will send you an invoice to pay for all of them in bulk, which will include instructions for how to make the payment via check or online.
2: Registering Your Players for USAU Memberships
- Collect all the information necessary to fill out a USAU Roster Form (name, DOB, email, etc). I’ve created a templated Google form that you can copy and then edit for your own team. It includes questions for all the info you’ll need. To copy it, click this link, then make sure you are signed in to your Google account. once you’ve done that, you’ll be able to “make a copy” of the form using the menu (3 little dots in the upper right corner). Please don’t use this actual form to collect your data, but just as a template to copy.
- Download and fill out your USAU Roster Form with all players (new and returning):
- For new players, no need to provide USAU #s; they will be created for you.
- For returning players, make sure to include their USAU #s.
- Make sure to include the name of the USAU-certified coach of your team
- If your team already exists in the USAU database, make sure you use the same team name on the form, so it will be easy for them to look up.
- Once you’ve completed your roster form, email it to Joel Pitney (joelpitney@gmail.com) and indicate whether you want Full $38 Youth Memberships (required if you are participating in the State Championships), or if you want $13 Affiliate Memberships (for teams that won’t participate in States):
- He will then send your roster to USAU who will create USAU memberships for new players and renew USAU memberships for returning players, and add them to your team.
- If your team is new, USAU will create one for you and add it with the league.
- If your team is returning, USAU will find your team and add it to the new league
- Once the team and memberships are created, we will notify you and provide usernames, passwords, and USAU#s for all new players. Make sure to send this information to all new players so they have access to their accounts.
4: Waivering Your Players
Once your players accounts have been created and you’ve distributed the necessary information to them, they or their parents (if they are under 18) must digitally sign the Waiver Release form on the USAU website. You can direct all players/parents to login to the USAU website and sign the waiver using these step-by-step instructions.
6: Filling Out Medical Authorization Forms for All Players
All players 17 years or younger must also fill out a Medical Authorization Form and give it to their coach/chaperone. Coaches/Chaperones must bring hard copies of these forms to all practices and tournaments.
